1. Introduction
Welcome to Louisville Pulmonary Care. By receiving medical services at our facility, you agree to abide by the following Terms and Conditions. Please read these carefully.

2. Patient Responsibilities

  • Patients must provide accurate and complete information about their medical history, medications, and current health conditions.

  • Patients must follow prescribed treatments and notify their provider of any changes in their condition.

  • Patients must respect office policies, staff, and other patients.

3. Appointments and Cancellations

  • Patients are responsible for arriving on time for scheduled appointments.

  • If an appointment must be canceled or rescheduled, at least 24 hours' notice is required.

  • Repeated missed appointments or late cancellations may result in discharge from the practice.

4. Financial Responsibility

  • Patients are responsible for all costs not covered by insurance, including copayments, deductibles, and non-covered services.

  • Payment is due at the time of service unless prior arrangements are made.

  • Patients are responsible for providing accurate and current insurance information.

  • Unpaid balances may be referred to a collection agency.

5. Insurance and Billing

  • Louisville Pulmonary Care will bill insurance companies as a courtesy, but patients are ultimately responsible for ensuring payment.

  • Any disputes regarding coverage must be handled directly with the insurance provider.

  • Statements for outstanding balances will be sent to the patient.

6. Privacy and Confidentiality

  • Patient information is protected under HIPAA regulations.

  • Patients have the right to access their medical records in accordance with applicable laws.

  • Medical records will not be released without written consent, except as required by law.

7. Prescription Policy

  • Medication refills require a minimum of 48 hours' notice.

  • Some medications may require an in-office visit before refills can be provided.

  • Controlled substances will be prescribed only as medically necessary and in compliance with state and federal laws.

8. Office Conduct

  • Abusive, aggressive, or inappropriate behavior toward staff or other patients will not be tolerated and may result in dismissal from the practice.

  • The use of cell phones should be minimized in the office to respect other patients and staff.

9. Emergency Care

  • Louisville Pulmonary Care does not provide emergency services. In case of a medical emergency, call 911 or visit the nearest emergency room.

  • Urgent issues should be addressed by calling our office during business hours.

10. SMS Messaging Consent

  • By providing a phone number, patients may consent to receive SMS messages from Louisville Pulmonary Care for appointment reminders, important health notifications, and periodic promotional messages. This will be for general use only and health related information will be disseminated through the HIPAA compliant patient portal.

  • Message and data rates may apply.

  • Patients may opt out at any time by replying STOP to any message received.

  • Standard carrier messaging fees may apply, and message frequency may vary based on service needs.

11. Changes to Terms and Conditions

  • Louisville Pulmonary Care reserves the right to update these Terms and Conditions at any time.

  • Patients will be notified of any significant changes as required by law.

By receiving care at Louisville Pulmonary Care, you acknowledge that you have read, understood, and agreed to these Terms and Conditions.
